Our health project

Four priorities for our neighbourhood

Our project responds to the needs of the Triolo district. Tap a card to discover the goals and actions of each priority.

Priority 1 Nutritional care for overweight and obesity Prevent, detect and support overweight and eating disorders, in children and adults.
Priority 2 Better care for chronic diseases Better screening and follow-up of cardiovascular-risk and respiratory diseases.
Priority 3 Better care for older people Support living at home and provide dignified end-of-life care.
Priority 4 Improving access to care for key groups Reaching out to vulnerable people: long-term-illness patients without a GP, students, emergencies.
90,3 % of residents over 17 have a GP
62,3 % breast-cancer screening (women 50-74)
21,1 % of patients have a long-term illness (ALD)
17 % covered by the solidarity health top-up
Our screening

Detect early to prevent better

We relay national campaigns and run our own screening, supported by our medical assistant and the team's coordinated work.

Cancer screening

Relaying national campaigns: breast, colorectal and cervical cancers.

Type 2 diabetes

Multi-professional screening protocol, BMI measurement and risk-factor collection.

Cardiovascular risk

Electrocardiogram (ECG), ankle-brachial index and 24-hour home blood-pressure monitoring (Holter), carried out by the team.

COPD & tobacco

Screening spirometry (HAS/GOLD criteria) and smoking-cessation support.

Sleep apnoea

Snoring, breathing pauses, waking up tired… a simple test (Epworth scale) helps tell whether screening is useful. The assistants fit the polygraph at home.

Childhood overweight

The "Mission : retrouve ton cap" programme supports children aged 3-12 who are overweight or at risk, free of charge, on a doctor's prescription.

Prevention check-ups

Systematic offer of prevention check-ups to eligible patients.

Sexual health & vaccines

STI screening, vaccinations (flu, Covid, pneumococcus, HPV) and contraception.

How our tests work

Tap a test to learn, in a few words, how it is done.

Electrocardiogram (ECG)

A few small electrodes (stickers) are placed on the chest, arms and legs. The device records the heart's electrical activity in a few minutes. It is quick and painless.

Spirometry

You blow hard into a mouthpiece connected to a device, following the instructions. It measures your breathing. The test lasts about 15 minutes and is not painful.

Ankle-brachial index (ABI)

Blood pressure is measured at the arm and ankle with a cuff and a small sensor. It checks the circulation in the legs. Painless, a few minutes.

24-hour blood-pressure monitoring

A cuff measures your blood pressure automatically for 24 hours, at home, day and night. You keep your normal routine. The medical assistant fits and explains the device.

Sleep polygraphy

To screen for sleep apnoea. The medical assistant sets up a small device with sensors (nose, finger, chest) at your home. You keep it for one night while sleeping at home. Painless.

Our care protocols

Harmonised practices across professionals

To care together consistently, the team gradually formalises shared protocols, planned under the Inter-professional Agreement (ACI).

Eating disorders

Coordinated detection and care of eating disorders.

Childhood and adult obesity

Prevention and follow-up pathway, from detection to dietary support.

Type 2 diabetes screening

Shared protocol for identifying and referring at-risk patients.

Pain management

Harmonised practices, especially for home care.

Anxiety and sedation at home

Support protocol for end of life and complex situations.

Cooperation with the CPTS

Cystitis cooperation protocol and COPD screening with the CPTS de la Marque.

Patient therapeutic education

Learning to live better with your illness

Free workshops led by local health professionals, to help you take an active role in your health.

Goals: understand the illness and its treatments, adapt your diet and recognise risky situations.

Infection prevention

Protect against germs: masks and clean hands

Simple actions that protect your health and others, especially during virus season.

Masks

A mask covers the nose and mouth and fits snugly. Wash your hands before putting it on and after taking it off, and change it as soon as it is damp.

Surgical mask

It holds back your droplets and protects others; it protects you from large droplets. Wear it when you are ill (cough, cold), in the waiting room, or if a carer asks.

FFP2 mask

It also filters fine airborne particles and protects you better. Useful for vulnerable people, when a virus is spreading widely, or on a carer's advice. It must be moulded well over the nose and chin.

Hand hygiene

Clean hands mean fewer germs passed on: before eating, after the toilet, after blowing your nose, when you get home.

Hand-sanitiser gel

On dry, not-dirty hands: a small blob of gel, then rub everywhere (palms, backs, between fingers, thumbs, nails) until dry, about 30 seconds.

Washing with soap

When hands are visibly dirty: wet them, lather everywhere for 30 seconds, rinse, then dry with a clean or single-use towel.

Our area

Our links with the CPTS de la Marque

A local health network (CPTS) brings together the professionals of an area to coordinate care. Our health centre works with the CPTS de la Marque on several concrete projects.

Diabetes education workshops

Run with the CPTS: a doctor, the pharmacist and the dietitian take part.

Care Access Service (SAS) cover

Taking part in organising cover across the CPTS area.

COPD screening

Cooperation protocol with nurses: spirometry and nicotine-substitute prescription.

Cystitis cooperation protocol

Delegated, coordinated management of simple cystitis.

Telemedicine

Telemedicine kits in the pharmacy, evolving towards nurse-assisted teleconsultations at home.

Childhood-obesity prevention

Prevention sessions in schools across the CPTS area.

Fighting undernutrition

Sessions with home-nursing services (SSIAD) in the area.

We want to keep developing these joint actions with the CPTS.

Teaching

A teaching health centre

We welcome and train future GPs. Supervision is provided by Dr Jonathan Favre, a University Teaching Practitioner (MSU).

All residents' consultations are supervised by Dr Favre, who is present at the practice and available at any time for direct supervision.

Level 1 resident

First placement in general practice. The resident first consults under direct supervision (the doctor is present), then increasingly independently. Every decision and prescription is validated with Dr Favre.

SASPAS — supervised autonomy

Supervised-autonomy placement in primary care. The experienced resident runs consultations alone. Cases are systematically debriefed and Dr Favre remains reachable at all times (indirect supervision).

Junior doctor

Consolidation phase of residency, close to independent practice. The junior doctor consults autonomously; Dr Favre supervises remotely, validates complex cases and remains responsible for care.
